Disputed Signature / Notary Record
Antonio Barnes:
“I did not sign this document.”
This Affidavit of Completeness contains a signature purporting to be Antonio Barnes and a notarial certificate identifying Kevin D. Burns, Illinois Notary Commission #972358.
Barnes states that the signature is forged and formally reported the disputed signature and notarization to the Illinois Secretary of State. Barnes alleges that Gregory Perkins placed or caused his signature to be placed on the document.

The document and Barnes’s government complaint are part of

or agency has made a final finding of forgery or notarial misconduct
unless such a determination is separately published.
